It's the pinwheel hat! Like the sabertooth mask, some people just didn't get it. I think it was a reward from a quest from Georgia. We have at least one topic and some polls about it if you're interested

Hmm. I looked at all the places the Pinwheel hat was mentioned and nobody seems 100% sure where they got it. Something to do with Georgia is all I’ve found. Maybe it was an Easter egg like the Turkey Leg and the Burned Hat and that weird egg hat thingy. (All of which I didn’t get, cough cough)
I wish I could just know where it came from, arg.

Singed Fireworks Hat - Special Anniversary quest (2017?) I was around for this, but was stuck with full quests!!!
Where you rigged up a fireworks display in the town square. Given as a reward.
Sabertooth mask - Sometimes found by a rock in the North Slurries
Lloyd's printing plate - Sometimes found by the entrance to the Isle Tunnels
